Launch Housing has an ambitious and singular mission to end homelessness. We aim to get people housed, keep them housed, and be an influential voice on ending homelessness. Formed in 2015 following a merger of two well-respected agencies - Hanover Welfare Services and HomeGround Services, we are a secular, independent provider of choice for clients and the "go to" organisation on homelessness for government, media, philanthropy, private donors and the community.

The Support for Families at Risk of Homelessness delivers intensive case management services to families and children who require long term support to successfully maintain suitable housing. The aim of the program is to tailor support so that families are prevented from entering the homelessness service system.

The role works with families and children for up to twelve months to deliver intensive support services based on a case-plan tailored to the needs of the family. The program assists families and children to positively engage in their communities of choice and, where appropriate, links families with resources in the community including education, training, health, employment and recreational opportunities. The role also focuses on the needs of children and actively and positively engages children with educational, health and support services.

The successful candidate will have tertiary qualifications in Social Work or a related discipline with experience in the social welfare sector, working with clients with complex needs and social policy/client support frameworks. The successful candidate will also have experience providing case management and support in an outreach capacity with an emphasis on practical assistance to maintain housing and an in depth understanding of the impact of homelessness on families.
