Complex Care Worker - Melbourne Street to Home

Two leading and very successful organisations in the homelessness and related sectors, HomeGround and Hanover, have merged to form a new agency, Launch Housing, with an ambitious agenda to achieve our vision of ending homelessness. Launch Housing is a strong, single, focused voice on homelessness driven by the values of Empowerment, Fresh Thinking, Agility, Leadership and Courage.

The MS2H position is focused on addressing a significant system gap which leads vulnerable people to become entrenched in chronic homelessness, and aims to end homelessness by providing rapid access to permanent, affordable and safe housing as a foundation together with intensive support to improve quality of life and community connections.

The Complex Care Worker position is funded under the DHHS funded Towards Home initiative, in response to the rising numbers of people sleeping rough in Melbourne. The practitioner will sit within the Melbourne Street to Home team who provide intensive, flexible and creative support to people who have experienced chronic homelessness.
